- The device(s) to be exposed in the chamber
- Directions on how you want your device exposed
- Pre-paid return shipping label (FedEx or UPS ONLY)- SEE SHIPPING NOTICE BELOW
- A signed disclaimer
- Confirmation of payment online
- Optional: Chain of Custody form
Device Performance Test Checklist
- YOU MUST FIRST COMPLETE A CLIENT REGISTRATION
- The monitor or passive device to be exposed in the chamber
- Power cord for each device (if necessary)
- A completed "KSU Radon Chamber Application for NRPP Performance Test"
- A partially completed "KSU DPT Reporting Form"
- Confirmation of payment online
- Pre-paid return shipping label (FedEx or UPS ONLY)- SEE SHIPPING NOTICE BELOW
- Instructions on how to start and stop the device. These are the steps you follow to operate your device (include any key or security access code). If passive devices, such as charcoal canisters or E-Perms, the client must send five devices. the client must perform any preparation of devices, such as weighing charcoal devices or measuring voltages on electrets, before shipping them to the KSU Radon Chamber.
Calibration Checklist
- YOU MUST FIRST COMPLETE A CLIENT REGISTRATION
- The monitor to be exposed
- Power cord for EACH monitor that is being calibrated
- A completed "KSU Radon Chamber Application for an Ecosense Radon Eye Pro Calibration"
- A partially completed "Radon Eye Pro KSU Calibration Exposure Form"
- Confirmation of payment online
- Pre-paid return shipping label (FedEx or UPS ONLY)- SEE SHIPPING NOTICE BELOW
- The client must perform any preparation of the device, such as updating the device, before shipping it to the KSU Radon Chamber

Packages MUST arrived BEFORE 12 NOON on weekdays.
PLEASE EMAIL your TRACKING NUMBER to radonchamber@ksu.edu. Otherwise delivery may be unsuccessful, adding significant delays to completing your Radon Chamber order. Send via FedEx or UPS ONLY to ensure timely delivery.
Please go online to buy pre-paid shipping labels vs. attempting to do so in-person at the store. We have found that certain UPS Stores and U.S. Postal Service branches do not allow customers to pre-pay for shipping labels due to liability concerns, creating challenges in getting your devices were they need to go.
We CANNOT use USPS STAMPS for return shipping.
